Transitional Rehabilitation Program

If you or your family member is dealing with a complex medical condition, the Transitional Rehabilitation Program offers a bridge from hospital to home or community living.

The Transitional Rehabilitation Program (TRP) is a high-intensity, inpatient rehabilitation facility. This nationally recognized, 48-bed skilled nursing facility is staffed by our interdisciplinary team of care providers.

If you have a disability or are recovering from illness, injury or surgery the TRP team can help you gain greater independence.

The philosophy of care integrates a holistic, mind-body approach within a healing environment, dedicated to providing you with whole-person care. This allows you to identify and work toward your goals, leading to increased independence and function.

In 2016, the TRP earned a five-star rating from the Centers for Medicare and Medicaid Services.

What it is

This team is made up rehabilitation specialists that include:

  • amputee clinic staff
  • aquatic therapy and fitness specialists
  • assistive technology specialists
  • nurse practitioners
  • physical medicine and rehabilitation physicians (physiatrists, on-site)
  • primary care practitioners (on-site)
  • pain specialists
  • physical and occupational therapists
  • psychiatrists (on-site)
  • psychologists and neuropsychologists (on-site)
  • skilled rehabilitation nurses
  • speech therapists
  • therapeutic recreation specialists

You’ll also have access to:

  • community services – Independent Living Skills, drivers training, Community Reintegration Program
  • comprehensive discharge planning
  • education
  • integrative therapies (aquatics, intensive fitness)
  • specialized therapies (TBI, stroke, spinal cord)
  • vocational services.

Good for treating

The Transitional Rehabilitation Program may be right for you if you have a diagnosis of: 

  • brain injury
  • cerebrovascular disorders (e.g., stroke, aneurysm)
  • multiple/complex amputations
  • neurovascular disorder (e.g., spinal stroke)
  • other complex neurological/neuromuscular disorders (e.g., multiple sclerosis, Guillain-Barre syndrome, Parkinson’s disease)
  • spinal cord injury.

What to expect

The Transitional Rehabilitation Program is set in the beautiful surroundings of Courage Kenny Rehabilitation Institute - Golden Valley campus. Here you’ll find a positive, supportive environment for healing.

Admissions staff will complete a pre-admission assessment to determine whether you or your family member requires the specialized programming TRP offers.

A team assessment begins when you are admitted to TRP.

You’ll meet regularly with staff to assess your needs and customize a plan of care and rehabilitation. You will set and regularly review goals with the members of your health care team.

Patients in TRP range in age from young adults to older adults. The average stay is 50-60 days.

Recreation opportunities

You can take advantage of various recreational activities. Depending on the season, activities may include:

  • adaptive sports (such as archery)
  • game nights
  • opportunities to observe Courage Kenny sports teams practice
  • outings to community events (such as the State Fair)
  • parties, picnics and barbeques
  • pet visits
  • support groups

Visitors

Visiting hours: 8 a.m.-10 p.m.

Please call ahead to find out when the person you are coming to see will be available. People in the Transitional Rehabilitation Program have frequent therapy appointments. Please do not come to visit if you are sick.

Parking and entrance

Free parking is available in lots on the southwest side of the building. Follow the signs for visitor parking. Please use the entrance marked Transitional Rehabilitation Program (TRP entrance on the map). The south parking lots (C and D) are the closest to this entrance.
